Enrollment at Taft Middle School

We have a convenient enrollment process that allows you to complete all the forms online. All forms should be sent electronically to Marsha McNeil at [email protected]

There are 9 items that need to be sent to Marsha McNeil to complete the enrollment process. Items 1-4 are fillable PDF's that need to be downloaded, completed, and then sent in. A copy of items 5-9 need to be scanned and sent in.

  4. SDUSD California Immunization Registry (CAIR) FORM (Click Here)
  5. BIRTH CERTIFICATE
  6. IMMUNIZATION RECORD
  7. LAST REPORT CARD
  8. PROOF OF RESIDENCY (copy of a bill @ SDGE, cable, water, lease agreement)
  9. A copy of student IEP, if applicable

All steps (1-9) will need to be completed.

Yearly Forms

The Forms listed below need to be completed yearly by each Taft Middle School Family. Please complete digital forms and email them to Marsha McNeil ([email protected]).

  1. Universal Form. This form can be downloaded and emailed to [email protected] (available below as a fillable form in English and Spanish).
  2. LCFF Form. This form can be completed and submitted online through PowerSchool Parent Portal.
